Agreement Form and Ordering

 Below are printable order and agreement forms to send in along with your chapbook
manuscript and payment. Order forms must be sent by fax or mail only. Shadow Poetry accepts personal checks, money orders, PayPal and cash from
US authors, and international money orders (USD), US cash or PayPal from international authors. If paying via
PayPal, all other materials
needed for chapbook publication must be emailed/mailed to Shadow Poetry and the order form must either be mailed or faxed.
Once the publication purchase total has been determined, click on the PayPal Payment
button below, and send the appropriate amount to Shadow Poetry. Once your manuscript and payment are received,
production on your publication will begin immediately. Please, keep in mind that should manuscripts not be sent via email, extra fees will apply.
Press the Chapbook Publication Order Form button to print out the Shadows Ink Chapbook Order and Agreement Form.
This form will allow you to choose your chapbook design, layout, fonts, etc. It also has chapbook
ordering and fee information, as well as an agreement you must complete for Shadows Ink/Shadow Poetry
to publish your chapbook. To order the Copyright Package deal, print out the appropriate form. Manuscripts may be
submitted via email or by mail (typing fees may apply if manuscript in hardcopy form only). If emailed, please refer to the email address below. Once you have completed the order form,
place it and your payment in an envelope addressed to (padded envelope if contains manuscript material):

1. Before clicking the button above you must be a PayPal member. PayPal allows you to pay anyone who has a valid email address
with a credit/debit card or checking account. Sign Me Up!!
2. After clicking the button above, you will be asked to sign in to PayPal to complete the transaction.
3. PayPal will ask you to fill in the total amount sending. After completing the transaction, be sure you press the "CONTINUE" link to bring you back to Shadow Poetry.

Chapbook Submission Checklist:
- Print and fill out the Shadow Poetry Order and Agreement Form.
- Gather any photos and/or artwork to be used in the publication process.
- Place all materials in a padded envelope or box (disks, photos, artwork, etc.) if sending by mail.
- Enclose payment for the full amount to publish your chapbook (unless paying by PayPal).
- Send package to the address indicated above.
